Acknowledgment of Risks

I,I, understand that participating in activities at [Insert Cat Café Name]
involves interacting with live animals and engaging in activities that may be inherently risky. I
acknowledge the potential risks and dangers that come with interacting with cats, including
but not limited to scratches, bites, allergic reactions, and the potential for emotional distress. I
also recognize the risks associated with the physical environment, such as slip and fall hazards.

Assumption of Risk

By signing this document, I voluntarily assume all risks, known and unknown, associated with
my visit to [Insert Cat Café Name], including any physical injury, illness, damage to personal
property, or other consequences that may arise from my participation in activities or interaction
with cats. I assume full responsibility for any and all risks of bodily injury, death, or property
damage, whether those risks are known or unknown.

Release and Waiver of Claims

I hereby release, waive, and discharge [Insert Cat Café Name], its owners, directors, employees,
volunteers, and agents (collectively, the “Releasees”) from any and all liability, claims, demands,
actions, and causes of action whatsoever, directly or indirectly arising out of or related to any
loss, damage, injury, or death, that may be sustained by me or any property belonging to me,
while participating in any activity or while in, on, or upon the premises where the activity is
conducted, except to the extent caused by the gross negligence or willful misconduct of the
Releasees.

Indemnification

I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs, including court costs
and attorney’s fees, that they may incur due to my presence in or upon the premises and my participation in activities,
except where such loss, liability, damage, or costs arise from the gross negligence or willful misconduct of the
Releasees.

Photo and Video Release

I grant [Insert Cat Café Name] and its agents the right to photograph, videotape, and record me and to use my
likeness in all forms and media for advertising, trade, promotional, and other lawful purposes without review or
further compensation.

Acknowledgment

I have read this Agreement, Waiver, and Release of Liability Form, understand its terms, and understand that I am
giving up substantial rights, including my right to sue. I acknowledge that I am signing this waiver freely and
voluntarily. The term of this waiver is indefinite and will remain in effect for all future visits to [Insert Cat Café Name].
